Participation Agreement for Online File Cabinet^®

I understand that by consenting to Online File Cabinet^® I am agreeing to electronic delivery instead of paper for all available documents from Empower Retirement. I have email and Internet access, so I will be able to receive email notifications and can review documents through the website. I understand Adobe Acrobat Reader 10 or higher is needed to view or print some of the documents. The Adobe Acrobat Reader can be downloaded, free, from www.adobe.com. All electronic documents may also be saved to my personal computer by downloading and saving the documents the way I would any other file from the Web. Statements will remain available through Online File Cabinet^® for a minimum of two years.

I agree to being notified at the email address I provide when documents are ready. When my email address changes, I'll update my new address at this website. I understand the email notifications will include a link to the website where documents are accessed.

I understand Empower Retirement will not charge for this service, but that I may incur costs associated with my Internet or phone service provider.

I understand my agreement to electronic delivery is effective immediately and will remain in effect unless and until either Empower Retirement or I change it. I can return to receiving paper documents by changing my agreement at this website. I understand that without changing my electronic delivery option, I can order a paper copy of a document at any time by sending a letter to Empower Retirement, P.O. Box 173764, Denver, CO 80217-3764, or by calling Client Service. Empower Retirement reserves the right to charge for additional paper copies.

I understand that in the event I change my email address (or other contact information), it is my responsibility to inform Empower Retirement by visiting this website and following the instructions provided on this website. I will need both my Login ID and PIN. If Empower Retirement is notified for the first time that the email address it has on file for me is invalid, it will notify me via mail that I need to update my address and view the latest document posted to the Online File Cabinet^®. Upon the second notification that my email address is invalid, my consent will be revoked and I will begin to receive paper copies. I understand that I may come back to this website and re-consent for Online File Cabinet^®.
